What:

The Art of Ribs BBQ Festival 2010 to include professional  6 BBQ vendors, up to 20 miscellaneous food vendors,  live music bands and entertainment, a 60×60 Beer tent and more!

Who:

Sponsored by the Lewiston/Niagara-On-The-Lake Rotary Club and held in conjunction with the 44th Annual Lewiston Art Festival which is sponsored by the Lewiston Council on the Arts

When:

Saturday, August 14, 2010 10AM – 10PM

Sunday, August 15, 2010 10AM – 8PM

Academy Park, Center Street, Lewiston, NY

Why:

We are seeking live bands and entertainment for the ART OF RIBS FESTIVAL 2010 !

How:

Complete the Music/Entertainment Application below and click on the submit tab.   The information will automatically transmit to the appropriate party.  If you have any questions prior to completing the application, feel free to call Elizabeth Kovacs at 716.282.2098 or email elizabethk@artofribs.com.  All prospective music and entertainment vendors will be contacted by email or phone confirming performance time slot, pricing and verification that the required documentation is in place.

Guidelines:

All music and entertainment vendors must adhere to the performance schedule including the precise setup and dismantle times. Equipment may be pre-staged in advance of the FIRST performers on stage on both days of the event, however, once the performances begin, only the time slotted for specific performers will be granted.

All Music & Entertainment Vendors must complete the APPLICATION below which will include your pricing information along with preference for performance times. All applications are due no later than June 30, 2010. Complete the form, print a copy for your records, and click on SUBMIT.

The final selection on all music and entertainment vendors will be made by July 12th and email confirmations will be sent to all finalists. Finalists will be required to provide a Certificate of Liability Insurance (details of which are outlined below) in addition to an invoice addressed to: ART OF RIBS 2010 c/o Lewiston/Niagara-on-the-Lake Rotary Club , 5117 Woodland Dr., Lewiston, NY 14092 and emailed to: leslies@artofribs.com.

Payment by the Lewiston/NOTL Rotary Club will be made on the day of performance by check.

A pdf footprint schematic outlining the layout of Academy Park and the location of the performance stage is available upon request.

Certificate of Insurance

 Yes, I will secure the required Certificate of Liability Insurance to include the following provisions:

Additional Insured: Name the Lewiston/Niagara-on-the-Lake Rotary Club, Lewiston, NY as additional insured on the Certificate.

Minimum coverage values:
A: General Liability
Each occurrence= $2,000,000
Damage to rented premises=$500,000
Personal + Adv Injury = $2,000,000
General Aggregate = $4,000,000
Products-Comp/OP Agg = $4,000,000
A : Automobile Liability
Combined Single Limit= $1,000,000
B: Excess/Umbrella Liability
Each Occurance= $5,000,000
Aggregate= $5,000,000

Additional Terms of Contract:

All music/entertainment vendors will supply their own tables, chairs, tents, power cords and equipment (unless otherwise specified).

Vendor will abide by all health, fire and festival regulations.

Vendor will supply the required and Certificate of Liability Insurance .

Vendor will hold harmless the Lewiston/Niagara-on-the-Lake Rotary Club, the Village of Lewiston, NY and/or any other party from any and all liability.

Vendor will be responsible for its own security and properties while on the Academy Park grounds.

Vendor is responsible for the removal of refuse to the supplied work space and promises to leave the space clean throughout the event and at the conclusion of the festival.

Should vendor representatives suffer any accident or injury, vendor authorizes any other participant in the Sponsor’s activities to administer first aid, treatment or service to assist representative. Vendor representatives understand that the aid, treatment or service, should it be provided, will be at their sole risk without any liability or risk to the Sponsor or the aid provider.

I understand that the Sponsor or other persons or entities may take photographs or create video or audio recordings of my activities with the Sponsor. I consent to grant and convey to the Sponsor in perpetuity all right, title and interest to any such photographs or recordings without royalty.

By my signature, or myself, my estate, and my heirs, I hereby release and discharge, and agree to forever hold harmless the Sponsor and its vendor partners, supervisors, officers, directors, agents, volunteers, servants and employees, from any and all causes of action and damages arising from or relating to my participation in this eventt, and any travel associated therewith.

I hereby certify that I am at least 18 years old and represent the vendor company or organization as an authorized party.
